The tube on the left says 500 ppm. The one beside it says 1000. Your sister-in-law brought back a 1450 from Dubai and swears by it. Your four-year-old has already decided, based on the cartoon on the front.
Short version first. "ppm" is just how much fluoride is in the paste. 1000 ppm means one milligram of fluoride in every gram of toothpaste. For most Indian children the evidence supports a paste in the 1000–1100 ppm range from the very first tooth, moving up to 1350–1500 ppm around age six. What decides fluorosis risk isn't the number printed on the box. It's how much paste goes on the brush, and how much of it your child swallows.

What does "ppm" mean on a toothpaste tube?
Parts per million. One part fluoride in a million parts of paste. A gram of toothpaste is a million micrograms, so the arithmetic stays easy: 1000 ppm = 1 mg of fluoride per gram of paste. A 1450 ppm paste carries 1.45 mg per gram.
Now hold that against the amount you squeeze out. A rice-grain smear is roughly 0.1 g of paste. A pea is roughly 0.25 g. So a pea of 1000 ppm paste puts about a quarter of a milligram of fluoride into your child's mouth, and a good part of that ends up in the basin rather than the child. Keep that scale in your head. It makes the fluorosis section further down a great deal less frightening.
When the box doesn't print a ppm figure
Plenty of Indian kids' pastes don't. Turn the carton over and read the ingredient percentages instead. The conversion is fixed chemistry:
- 0.76% sodium monofluorophosphate ≈ 1000 ppm fluoride
- 0.221% sodium fluoride ≈ 1000 ppm fluoride
- 1.1% sodium monofluorophosphate ≈ 1450 ppm
- 0.32% sodium fluoride ≈ 1450 ppm
- 0.11% sodium fluoride ≈ 500 ppm
If neither compound appears anywhere in the ingredient list, the paste has no fluoride in it. Whatever the front of the pack says about "dental protection".
500 vs 1000 vs 1450 ppm: which one does your child need?
The 500 ppm pastes are the ones I'd think hardest about. They get sold as the gentle, sensible middle path for little children, and that framing lands beautifully on a worried parent standing in a medical store at 8pm. But when researchers pooled the trials on fluoride toothpaste, the clear cavity-prevention benefit showed up at 1000 ppm and above. At roughly 500 ppm, measured against a fluoride-free paste, the effect wasn't convincing. Your child still swallows a little paste. You just don't get much protection in return.

How much to squeeze at each age
This is the lever that matters. Every toothpaste advert ever made quietly works against it. Those photographs of a fat ribbon of paste curling across the whole brush head? That's close to a full gram. Four times a pea, roughly, on a mouth a quarter the size.
- First tooth to 3 years: a thin smear, the size of a grain of rice. You should barely see it.
- 3 to 6 years: a pea. A real garden pea, not a cashew.
- 6 years and up: a pea, or a 1 cm strip once they're using a larger brush head.
Two small mechanics are worth getting right. You squeeze the paste, not your child. A five-year-old holding the tube produces a ribbon every single time, and they will keep squeezing while you say no. Then press the paste down into the bristles rather than leaving it sitting on top like icing. A toddler licks the top stripe off before the brush gets near a tooth. Paste worked into the bristles reaches the teeth.
The nightly brushing routine, step by step
If you change one thing after reading this, make it the night brush. Saliva flow drops while your child sleeps. That's when fluoride sitting on the enamel does its best work, and when leftover milk or half a biscuit does its worst. It's also why what happens after the last night feed matters as much as the brushing.
- Brush twice a day. Morning whenever it fits your chaos. Night is the non-negotiable one, after the last milk or food.
- You dispense the paste. Rice grain under three, pea from three to six. Check the amount again every few months. Children grow; peas don't.
- Press it into the bristles so it can't be licked off in one go.
- Two minutes, in sections. Thirty seconds each: upper outer, upper inner, lower outer, lower inner. Small circles, bristles tilted towards the gumline.
- Don't skip the three boring spots. The chewing grooves of the back molars, the inside of the lower front teeth, the gumline of the upper front teeth. That's where decay starts in small children.
- Spit out. Do not rinse. No water, no mouthwash, no cup. Spitting takes off the excess paste. Rinsing takes off the fluoride that was about to do its job.
- Nothing after the night brush except plain water.
- You do the brushing until about seven or eight. Let them have their go first, for the independence, then you go in and do it properly. Stand behind them with their head tilted back against you. It beats fighting them face-on.
If two minutes currently sounds like fiction in your house, that's normal. The brush design and the position you hold them in do more of the work than willpower does.
So what about fluorosis? An honest answer for Indian parents
Dental fluorosis happens when too much fluoride reaches teeth while they are still forming under the gum. The window runs from birth to about six years, and the upper front teeth, the ones everybody notices, are most sensitive somewhere around 15 to 30 months. Once a tooth has come through, that tooth can't develop fluorosis.
At the mild end it looks like faint white flecks or fine lines on the enamel, often so subtle that a dentist has to point them out to you under a good light. It looks nothing like the mottled, stained, pitted teeth in the photographs you may have seen from Nalgonda or western Rajasthan.
Here's the distinction Indian parents actually need, and most international advice skips it. Severe fluorosis in India is a drinking water problem, not a toothpaste problem. The BIS drinking water standard puts the acceptable limit at 1.0 mg of fluoride per litre, with 1.5 mg/L permissible only where there's no alternative source. High-fluoride groundwater above those limits is documented across districts in Rajasthan, Gujarat, Telangana, Andhra Pradesh, Karnataka, Tamil Nadu, Haryana, Punjab, Uttar Pradesh, Madhya Pradesh, Bihar, Assam and parts of Maharashtra. A child drinking a litre of 2 mg/L borewell water takes in 2 mg of fluoride that day. A pea of 1000 ppm paste, mostly spat out, contributes a small fraction of a milligram.
So find out what's in your water first. Then worry about the tube. If you're on borewell or untreated municipal supply anywhere in those belts, get the water tested at a NABL-accredited lab. It costs very little and it will tell you more than reading toothpaste cartons ever will. And if your household drinks RO water, reverse osmosis strips out most of the fluoride too. When to see a dentist
Book an appointment, rather than waiting for the next check-up, if you notice any of these:
- Chalky white or brown patches along the gumline of the upper front teeth. Parents often read these as fluorosis. Early decay looks very similar, and the two need completely different responses.
- Any hole, dark spot, chip or tooth that has changed colour.
- Your child complains of pain or sensitivity with cold or sweet things, or chews only on one side.
- Bleeding or swollen gums that don't settle within a week of proper brushing.
- Your child has eaten a meaningful amount from a toothpaste tube. Ring your paediatrician or dentist the same day.
- You live in a known high-fluoride area and your water has never been tested.
And if you haven't been yet: the first dental visit should happen by your child's first birthday, or within six months of the first tooth appearing, whichever comes sooner. Most of that first visit is talking to you rather than treating them. That's the reason to go early.

Probably not. When trials of fluoride toothpaste were pooled, the clear cavity-prevention benefit appeared at 1000 ppm and above, while pastes around 500 ppm were not convincingly better than fluoride-free ones. Most current paediatric dental guidance uses 1000–1100 ppm from the first tooth, with the amount kept to a rice-grain smear under three. Control the quantity rather than dropping the concentration.
How much toothpaste should a 1-year-old use?

Will my child get fluorosis from swallowing toothpaste?
Mild fluorosis — faint white flecks a dentist may have to point out — is possible if a young child regularly swallows large amounts of paste while the permanent teeth are forming, which is roughly birth to age six. A correctly sized rice grain or pea, mostly spat out, contributes very little. In India, severe fluorosis comes overwhelmingly from high-fluoride drinking water, not toothpaste.
Should my child rinse with water after brushing?
No. Spit out the excess paste and stop there. Rinsing with water or mouthwash washes away the thin layer of fluoride that was about to sit on the enamel and do its work, which is most valuable overnight when saliva flow drops. Teach spitting from around age three; before that, wipe the mouth rather than rinsing. Nothing but plain water after the night brush.
How do I find the fluoride ppm if it isn't printed on the pack?
Read the ingredient percentages on the carton. Roughly: 0.76% sodium monofluorophosphate or 0.221% sodium fluoride equals about 1000 ppm; 1.1% monofluorophosphate or 0.32% sodium fluoride equals about 1450 ppm; 0.11% sodium fluoride is about 500 ppm.
